Interventional Spine, Inc. announced the completion of another successful surgery with the unique PERPOS Percutaneous Cervical System for the treatment of neck pain.

The surgery was performed at the Mercy Medical Center in Redding, CA, by Dr. David Bradley Jones, MD. The Impact Of Commonly Requested Cosmetic Surgery On Body Dysmorphic Disorder Symptoms

A new study finds that while many who suffer from body dysmorphic disorder (BDD) seek cosmetic procedures, only two percent of procedures actually reduced the severity of BDD. Despite this poor long-term outcome, physicians continue to provide requested surgeries to people suffering from BDD. The study was recently published in Annals of Plastic Surgery.

Katharine A. Phillips, MD, is the director of the body image program at Rhode Island Hospital and a co-author of the paper. Phillips says, “BDD is a psychiatric disorder characterized by preoccupation with an imagined or slight defect in appearance which causes clinically significant distress or functional impairment. A majority of these individuals believe they have an actual deformity that can be corrected by cosmetic treatments to fix these perceived defects rather than seeking psychiatric intervention.”

Phillips and her co-author, Canice Crerand, PhD, of The Children’s Hospital of Philadelphia, reported in previous studies that BDD appears relatively common among individuals who receive cosmetic surgery, with reported rates of 7 to 8 percent in cosmetic surgery patients in the United States. Even with the high frequency of those with BDD seeking and receiving cosmetic procedures, few studies have more specifically investigated the clinical outcomes of surgical and minimally invasive cosmetic treatments, such as chemical peels, microdermabrasion, and injectable fillers).

In their new study, the researchers report that in a small retrospective study of 200 individuals with BDD, 31 percent sought and 21 percent received surgical or minimally invasive treatment for BDD symptoms. Nearly all of these individuals continued to have BDD symptoms, and some actually developed new appearance preoccupations. They also note that in a survey of 265 cosmetic surgeons, 178 (65 percent) reported treating patients with BDD, yet only one percent of the cases resulted in BDD symptom improvement. Phillips, who is also a professor of psychiatry and human behavior at The Warren Alpert Medical School of Brown University, says, “These findings, coupled with reports of lawsuits and occasionally violence perpetrated by persons with BDD towards physicians, have led some to believe that BDD is a contraindication for cosmetic treatment.”

The researchers found that the most common surgical procedures sought were rhinoplasty and breast augmentation, while the most common minimally invasive treatments were collagen injections and microdermabrasion. Three quarters of all the requested procedures involved facial features. The findings also indicate that more than a third of patients received multiple procedures.

In terms of long-term outcomes from procedures, only 25 percent of the patients showed an improvement in their appraisal of the treated body part and showed a longer-term decreased preoccupation. However, as noted by co-author Crerand, “Only two percent of surgical or minimally invasive procedures led to longer-term improvement in overall BDD symptoms.”

The researchers also found that when treatment was sought, 20 percent of the procedures were not received. Cost was the most common reason for not receiving the requested procedure (30 percent), followed by physician refusal to perform the procedure (26 percent).Their findings also indicate that physicians were significantly less likely to refuse a surgical or minimally invasive treatment than other procedures (dermatological, dental and others). Phillips says, “This suggests that many surgeons were not aware of the patient’s BDD or do not consider BDD a contraindication to treatment. In a survey of 265 cosmetic surgeons, only 30 percent believed that BDD was always a contraindication to surgery.”

The researchers conclude, “This study provides new and more detailed information about receipt and outcome of surgical/minimally invasive procedures, and the findings indicate that there is a clear need to further investigate this topic in prospective studies. In the meantime, physicians need to be aware that psychiatric treatments for BDD such as serotonin reuptake inhibitors and cognitive behavioral therapy appear to be effective for what can be a debilitating disorder.”

Also involved in the study is William Menard of Alpert Medical School. The study was funded through a grant from the National Institute of Mental Health.

Risks, Benefits Of Heart Valve Replacement Technique Not Fully Understood

A newer, less invasive method of heart valve replacement shows promise and may be appropriate for patients who cannot tolerate traditional open heart surgery, but research is needed to understand its potential risks and benefits, according to a new study funded by HHS’ Agency for Healthcare Research and Quality (AHRQ).

The report finds that percutaneous heart valve replacement, a procedure in which a replacement valve is implanted through a catheter rather than by open-heart surgery, is a realistic option for some patients with heart valve disease, especially older or sicker patients. The report found that percutaneous heart valve replacement may be a safe and effective alternative to open heart surgery, especially in the short term, for those patients. However, information is lacking on the potential long-term benefits and risks of this procedure, particularly compared with open heart valve replacement surgery. The report did not conclude that any of the seven valves studied is safer or more effective than another.

The report was produced by the Duke Evidence-based Practice Center for AHRQ, a leading federal agency conducting comparative effectiveness research. Results of the report were published online today in Annals of Internal Medicine. Heart valve disease a narrowing of the heart valve is blamed for approximately 20,000 deaths a year.

“As the U.S. population ages, the number of patients with heart valve disease will increase, so more Americans will need options for safe, effective heart valve replacement,” said AHRQ Director Carolyn M. Clancy, M.D. “This report identifies what is known about this emerging procedure and where the research is lacking to give patients more options about their treatment.”

Conventional heart valve replacement is a well-established procedure, with approximately 95,000 open heart surgeries performed per year for patients with severe narrowing of the heart valve. However, as many as one-third of patients with severe heart valve disease are considered too high risk for open heart surgery, in which patients typically are under general anesthesia for three to six hours, are hospitalized for 5 to 6 days, and require recovery time of 12 weeks. These patients would likely benefit from a less invasive option.

The report found that percutaneous heart valve replacement is a good option for patients at high risk for traditional surgery because of advanced age or illness. Approximately 92 percent of patients who received a percutaneous valve survived the procedure; of those, 86 percent survived for at least 30 days, the report found.

Catheter-based valve implantation was first reported in 2002. Just one device, the Medtronic Melody Transcatheter Pulmonary Valve and Ensemble Delivery System, currently has approval from the U.S. Food and Drug Administration. This device was approved in January 2010 under the Humanitarian Device Exemption program. Percutaneous heart valve replacement surgery has been available in Europe for nearly 2 years.

The report identified 62 published studies representing a total of 856 patients, as well as additional studies not yet published. However, researchers were unable to make direct comparisons between percutaneous valves and traditional surgical replacement because of differences between patient groups receiving the treatments. Clinical trials that may allow direct comparisons are currently underway in the United States.

Percutaneous Heart Valve Replacement is the newest technical brief from AHRQ’s Effective Health Care Program. Technical briefs are rapid-turnaround reports that summarize key issues regarding emerging treatments, highlighting where more research is needed, and where research may be sufficient to warrant a full systematic comparative effectiveness review. The Effective Health Care Program represents a leading federal effort to compare alternative treatments for health conditions and make the findings public to help doctors, nurses, pharmacists and others work together with patients to choose the most effective treatments.

In conjunction with the new report, AHRQ will soon publish plain-language summary guides about heart surgery for patients, clinicians and policymakers.

Excaliard Announces Positive Data From Phase 2 Clinical Trial Of Its Anti-Scarring Drug, EXC 001

Excaliard Pharmaceuticals, Inc., announced positive results from its Phase 2 clinical trial of EXC 001, a new chemical entity for reducing fibrosis. Results showed that EXC 001 treatment significantly reduced scar severity in subjects undergoing an elective abdominoplasty compared to placebo.

In the randomized, double-blind multicenter study conducted in the U.S., EXC 001 or placebo was administered intradermally adjacent to the surgical incision in patients who were undergoing elective abdominoplasty surgery. In this study multiple measurements of scarring were performed with analysis conducted at 12 weeks post-surgery. Data reported today showed statistically significant improvements in scarring for EXC 001 compared to placebo in both physician and patient reported outcomes (p values ranged from 0.017 to 0.002). In addition, EXC 001 was well tolerated with no clinically important adverse events. The final analysis for this study will be conducted 24 weeks after surgery and these data will be available in the second half of 2010.

“The prevention and treatment of unsatisfactory skin scarring remains an important unmet medical need,” said Lincoln Krochmal, MD, CEO & President of Excaliard Pharmaceuticals. “Unsatisfactory skin scarring can be associated with various dermatologic conditions such as hypertrophic scars and keloids, elective and reconstructive surgery, traumatic wounds and Caesarian sections. Surgeon, Age Is Relative When Considering Surgical Intervention

Age alone should not determine surgical risk in a person age 80 or over, according to expert commentary from Mark Katlic, M.D., director of Thoracic Surgery at Geisinger Wyoming Valley Medical Center in Wilkes-Barre, Pa.

Dr. Katlic’s commentary complements a McGill University Health Science Center trend study, appearing in this week’s edition of the Canadian Medical Association Journal (CMAJ). The study finds an increase in coronary artery bypass grafts (CABGs) and percutaneous coronary interventions (PCIs) with better results in people previously believed to be too old for these procedures.

Physiologic age can be very different from chronologic age, according to Dr. Katlic, a thoracic surgeon and geriatric surgery specialist.

“I’ve seen 95-year-old patients who are in far better health than patients 20 years younger,” noted Dr. Katlic. “In these patients, a surgical intervention (that is indicated) will almost always improve and not diminish – quality of life.”

As baby boomers bend life expectancy curves, an increasing number of them will be considering aggressive surgical procedures in their advanced years.

“We need a simple and reliable test, the elusive object searched for by those of us in the field, sort of the Holy Grail of geriatric surgery,” said Dr. Katlic, “to help elderly patients and their doctors better estimate the risk of undergoing a surgical procedure.”

Editor of several definitive geriatric surgery textbooks and a frequent national speaker on geriatric surgery topics, Dr. Katlic suggests that appropriate surgical intervention in the elderly usually decreases medical costs. He said that despite related co-morbidities, such as diabetes, the intervention generally reduces condition-specific hospital admissions and readmissions.

“This is a topic that demands more research,” he noted.

Led by a research team from McGill University Health Centre (MUHC) in collaboration with Hpital du Sacr-Coeur de Montral (HSCM), Universit de Montral (UdeM) and the Jewish General Hospital in Montreal, Quebec, the study involved 29,750 people age 80 years and older. It is one of only a few studies reporting long-term mortality trends in people within this age group opting for heart surgery and medication following an acute myocardial infarction.

Psychologist Links Reality TV With Teen Cosmetic Surgery

Teenage years have long been linked with a heightened concern with appearance. Some reality TV shows take full advantage and tout happiness as just a nip/tuck away. A Rutgers-Camden psychologist has found that teens fond of these kinds of programs are more likely to join the millions who go under the knife each year. For bodies – and minds – still in development, these drastic decisions could have implications way after prom.

Charlotte Markey, an associate professor of psychology at Rutgers-Camden, with husband Patrick Markey of Villanova University, recently published research on this topic in the academic journal Body Image.

“When we think of cosmetic surgery, we don’t think of it as a lifetime issue. There is lots of pressure to look a certain way and I don’t blame them for succumbing; we’re all guilty of feeling vulnerable. But what young men and women think of their bodies now will culminate over time and contribute to their overall health,” notes the Rutgers-Camden psychologist. “What troubles me is that there’s no conclusive data that cosmetic surgery even makes people happier, what has been documented is that it makes repeat customers.”

The wife-and-husband team surveyed nearly 200 participants with an average age of 20 on their immediate responses to an ‘extreme makeover’ program or a show on home improvement – incorporated specifically to mask the intent of the study. Both men and women were included in the study and the procedures examined were ones either gender could pursue.

As the Rutgers-Camden researcher suspected, women were more likely to want cosmetic surgery than men and viewers of the cosmetic surgery show were more inclined to consider the procedure for themselves than those who didn’t tune in. What still shocks Markey are the handwritten responses to the cosmetic surgery show, including comments like “inspirational” and “I saw an unhappy girl get her dreams.”

This saddens Markey because outward appearance seems to be the sole avenue to self satisfaction and this road, she believes, is circular. “If plastic surgery makes you feel better about yourself, then why do you keep getting it done?” she asks. “This mindset is very similar to that of an anorexic wanting to lose just five more pounds.”

While ABC’s Extreme Makeover, which led to the Extreme Makeover-Home Edition, was cancelled in 2007, it sparked the development of several other similarly themed shows like Fox’s The Swan, the drama Nip/Tuck, MTV’s I Want a Famous Face, E’s Dr. 90210 and Oxygen’s recent Addicted to Beauty. The impact of reality television as a new media influence – regardless of topic – also raises many questions about what is being portrayed to viewers as real and indicative of everyday life.

“There is a cultural context to never be satisfied with our physical selves. It’s the rare person who is either completely oblivious or has developed such a strong counter message to not be affected, “notes Markey, the mother of a four-year-old boy and a three-year-old girl. “We need to teach children to be critical of the messages we’re receiving and tell them positive things now to foster self-esteem.”

Markey brings these cultural dialogues into her own home by explaining to her young children why Barbie is banned. “I tell them ‘no one you will ever meet in real life will ever look like her’” she says. “There are so many messages out there telling us that we need to fix ourselves, but at an early age we need to tell our kids: ‘I love you just the way you are.’”

She teaches the courses Psychology of Eating; Health Psychology; Psychology of Health and Happiness; Psychology of Adolescence; and Research Methods at Rutgers-Camden.
