Flawless Skin Tops Color Cosmetics For Women As Makeup Priority

| September 23, 2010

A new national survey suggests a trend towards a less “made up” look for American women. Above all, the survey finds women expect their cosmetics to deliver flawless skin and sun protection, above fashion-forward colors.

Commissioned by Neutrogena Cosmetics, the #1 dermatologist recommended brand, and executed by Harris Interactive, the survey explores the complex relationship American women have with their cosmetics.

“As a professional makeup artist I’m pleased to learn that American women are tuned into what so many professionals already know – beautiful skin is essential to any look,” said Amy Oreasman, celebrity makeup artist to Neutrogena Cosmetics Brand Ambassador Hayden Panettiere. “It’s the canvas women work with everyday and when it looks great so will everything else.”

Among the Neutrogena Cosmetics survey highlights:

– When asked if they could only own one cosmetic product, over half of young women, 58%, say they want a product that delivers flawless skin tone

– Over two-thirds of women, 68%, say they would be willing to give up something for a year to have flawless skin — even chocolate (51%)

– Nearly half of women, 46%, say they view using or wearing cosmetic products and makeup as an extension of their overall skincare routine

– More than half (56%) rely on cosmetics with SPF to provide protection from harmful UV rays

The key benefits women want from their cosmetic products and makeup are directly related to beautifying their skin’s appearance. Although flawless skin tone was the leading response for all women (58%), over half of older women (55+), 51%, report the key benefit they want is to minimize the look of fine lines and wrinkles. However, regardless of age, these skin-related benefits were chosen ahead of more traditional cosmetic benefits including elongate lashes, 8%, make lips look full, 3%, and cheekbone definition, 2%.

Commenting on the Neutrogena Cosmetics survey, Cara Robinson, Group Brand Director, Neutrogena Cosmetics said, “The survey confirmed the desires we hear from women to provide more advanced cosmetic solutions that improve their complexion and help achieve that flawless look. While color cosmetics add drama, we’re thrilled to see women embracing their own beauty and choosing products that work harder to help deliver a perfect complexion.”

Even as women embrace the natural look, makeup application remains an integral part of many women’s morning routines. Nearly half, 42%, say if they had to choose between makeup and breakfast when pressed for time before work, they would choose to apply makeup.

Women also recognize the dangers of UV rays and look to their cosmetics to help prevent future skin damage. The majority of women, 56%, say they wear cosmetic products and makeup with SPF protection and older women are more likely to wear SPF cosmetic products and makeup year round, 72% vs. 50%.

Neutrogena Cosmetics offers a range of foundations, powders, concealers and brightening products that leave skin more beautiful even after women take them off. The NEUTROGENA HEALTHY SKIN and NEUTROGENA MINERAL SHEERS lines contain anti-oxidants to rejuvenate the complexion for skin that looks young, radiant and healthy. The NEUTROGENA SKINCLEARING line doesn’t just cover breakouts; it works quickly to treat them with Salicylic Acid, enhanced by MICROCLEAR technology. Nearly all Neutrogena Cosmetics also contain SPF protection of at least SPF 15.

Survey Methodology:

This survey was conducted online within the United States by Harris Interactive on behalf of Neutrogena Cosmetics from August 9-11, 2010 among 1,018 adult women ages 18 and older. This online survey is not based on a probability sample and therefore no estimate of theoretical sampling error can be calculated.

New bath ballistics from Lush

| September 22, 2010

Mo Constantine, one of the Lush Co-Founders, first created the original bath ballistic over 20 years ago for her children – she wanted something to put in the bath to successfully send them off to sleep.

Still a Lush best seller – with A-list fans including Angelina Jolie, Gwyneth Paltrow, Julia Roberts, Kylie Minogue, Helena Christensen and Sarah Jessica Parker – each one is formulated with the finest ingredients and essential oils to cleanse both the mind and body. Simply drop into a ready run, warm bath and watch as it fizzes around the tub with abandonment, emitting its goodness into the water.

Today, this concept has been reinvented. Together with her youngest son Jack, they have created a new version that combines two of Lush’s greatest innovations: the bubble bar and the bath ballistic. Layering these two materials together enables totally different effects to happen in the water at once; the top layer of ballistic mixed with shavings of bubble bar languorously froths around the tub, leaving behind a trail of skin softening foam and bubbles. You then get to the inner ballistic layer that starts to frantically fizz around, shooting different colours into the froth and spinning around like an indoor firework!

The visual effect is simply stunning and makes for a mesmerising bath time experience. The only thing is you wont want to get in and disturb all the pretty patterns!

Mrs Whippy (2.50)
A hug in a tub, drop this ballistic in the water and sink into a frothy whirlpool of strawberry scented, softening foam. Slowing fizzing around the tub, the external layer leaves behind a trail of white foam, while the inner layer delivers a shot of pink both of which mingle together for a delicious, strawberry ice-cream effect. Just dont be tempted to stick a flake in it! Vegan.

Dragons Egg (2.95)
Pop one of these mythical beauties into the tub and marvel as it swirls around and around leaving a trail of firey orange foam in its wake. Grapefruit, jasmine and lemon oil scent the bath, to uplift and refresh a tired mind while popping candy provides the snap and crackle! This is the only one with three layers; the bubble bar shavings are in the outer layer, the ballistic mix explodes from the middle layer and then theres a small centre of pure golden lustre. This swirls amongst all the orange to leave a delicate shimmer to the skin. Vegan.

Twilight (2.95)
Immerse yourself into a nice warm bath and drop in this soothing, meditative lavender and ovaltine bath ballistic. Become entranced as it serenely fizzes around you, enveloping you in a relaxing cloud of fragrance. Starting off a beautiful, hazy pink colour the slow fizzing ballistic froths around to imitate the setting sun, leaving skin softening bubbles looking a little like clouds. The ballistic then gets down to its inner layer, fizzing frantically round the tub and injecting trails of blue with the pinkness, turning the water the deep indigo of a twilight sky. And for those with vampire obsessions, this will help you sleep soundly at night! Vegan.

Fizzbanger (2.95)
Whizz! Pop! Fizz! Bang! Throw one of these candy apple and cinnamon scented Fizzbangers into your bath and oooh and ahhh as yellow and blue frothy trails swirl around and around, dart one way and then the other, all twirling together to form green streaks across the water. Catch these dream-like formations if you can, theyre like a Catherine wheel in your tub with popping candy providing extra fizz, pop and crackle! Cinnamon leaf and petitgrain oil can also help to warm the circulation, relieve tired muscles and soothe the mind. Its like Bonfire night has been brought to your bathroom! Vegan.

These new bath ballistics are available in all Lush shops.

Orthopaedic Foot And Ankle Surgeons Changing Lives In Vietnam

| September 21, 2010

 0 posts

This year marks the 9th Annual Surgical Outreach Project to Vietnam by members of the American Orthopaedic Foot & Ankle Society (AOFAS). Since the first project in 2002, the AOFAS surgeons have transformed the lives of more than 600 Vietnamese children and adults with lower extremity deformities and disabilities through corrective surgery. Due to the cost, these patients would likely never receive the care or surgery provided by the AOFAS members. Two teams of visiting AOFAS members volunteer over a four week period traveling to villages and towns in northern and central Vietnam and work side by side in clinics and operating rooms with Vietnamese orthopaedic surgeons and residents as part of an educational exchange. The AOFAS members volunteer their time, pay their own travel expenses, and in the end their own lives are changed by the experience.

Naomi Shields, MD is a veteran on these trips having participated annually for the past nine years. “I have seen a lot of changes over the 9 years, especially in the traffic and the high rise buildings. What has not changed has been the friendly welcoming nature of the Vietnamese people, and the overwhelming need especially for the poor and disabled. What we do is a small drop in the bucket of need but for the 600 patients AOFAS surgeons have operated on, that drop has been life changing. I hope we can continue with this project and expand to other countries,” said Dr. Shields.

The AOFAS Overseas Outreach Project to Vietnam supported by the Orthopaedic Foot & Ankle Outreach & Education Fund (OEF) is a partnership initiative with the Prosthetics Outreach Foundation (POF), which has been providing prosthetic limbs to Vietnamese children and adults since the 1990s but until 2002 lacked an advanced medical services component for patients in need.

As a complement to the mission of the POF, the AOFAS project offers free corrective surgery to patients with lower limb deformities and disabilities caused by polio, cerebral palsy, clubfoot, trauma or amputation. Many patients are from impoverished areas often without access to care and/or are unable to afford advanced medical services. Over the nine years of the project, AOFAS surgeons have seen more than 2,000 patients in clinics and operated on more than 600 patients.

The AOFAS members share new technologies with Vietnamese orthopaedic surgeons and in return are exposed to foot and ankle conditions that many have only read about as classic textbook cases, such as severe untreated clubfoot. In addition to the surgical component, the AOFAS co-sponsored an educational conference on Surgery of the Lower Extremity in Hanoi. The visiting surgeons participated in the educational exchange which was attended by more than 150 Vietnamese orthopaedic surgeons. This cultural and professional exchange further enhances the experience for both AOFAS members and Vietnamese orthopaedic surgeons, while the educational component of the mission creates opportunities for local surgeons to learn directly of new surgical advancements in the treatment of foot and ankle disease and deformities.

Participating in this year’s project were: Judith Baumhauer, MD, Rochester, NY; A. Holly Johnson, MD, Boston, MA; Steven Sheskier, MD, NY, NY; Naomi Shields, MD; Wichita, KS and Saul Trevino, MD, Columbia, MO. During the four week project, the surgeons traveled to hospitals in Thai Nguyen (northwest of Hanoi); Thanh Hoa, (coastal city south of Hanoi); Dien Bien Phu (mountainous region along the Laotian border), and worked at both an orthopaedic and rehabilitation center and Viet Duc University Hospital in Hanoi.

The American Orthopaedic Foot and Ankle Society is a professional society of more than 1,800 orthopaedic surgeons specializing in diagnosis, care and treatment of patients with disorders of the musculoskeletal system of the foot and ankle. The Surgical Outreach Project to Vietnam is supported by the Orthopaedic Foot & Ankle Outreach & Education Fund.

UT Southwestern Plastic Surgeons Offer Latest Fat-Busting Laser

| September 21, 2010

 0 posts

Stephanie Martinez had one of the most universal desires – to shed a few stubborn inches from her waist, hips and thighs – without harsh diets, needles or other surgical interventions that require staying at home and taking time off from work.

So she agreed to help UT Southwestern Medical Center plastic surgeons evaluate Zerona, a new type of body-contouring laser that moves effortlessly across the outer skin, disrupting fat cells so they flush out of the body naturally.

After lying on her back for 20 minutes, then on her stomach for another 20 minutes while an array of light waved over her waist, hips and thighs, Ms. Martinez, 36, was able to walk out of the room and immediately return to work. Compared to traditional liposuction, Zerona requires no incisions, and there are no burning aftereffects, as with other types of lasers.

“There was really no downside. You can do it during your lunch hour,” she said. “It didn’t hurt or anything. That’s what I loved about it. There’s no invasive surgery or poking or injecting. You just lie there and relax.”

The laser, just approved by the Food and Drug Administration to be both safe and effective for noninvasive body slimming, can be used on the waist, hips and thighs and may also work for other applications, such as male breast reduction or neck and arm flab. The procedure requires six, 40-minute sessions over a two-week period.

“Zerona is really for helping you fit into your clothes,” said Dr. Jeffrey Kenkel, vice chairman of plastic surgery and director of the Clinical Center for Cosmetic Laser Treatment at UT Southwestern.

Zerona does not supplant traditional liposuction for those needing a more significant contour change, so good candidates need to be close to their ideal body weight and have good skin quality to see good results, he said.

“I lost inches on both thighs and on my hips – about 2 inches on my hips. My stomach area is my real problem area, and I lost nearly 3 inches,” Ms Martinez said.

The low-level lasers cause fat cells to lose the fat, Dr. Kenkel explained. The body then disposes of it naturally through absorption or excretion.

“We’re a clearinghouse for technology, and we’ve been excited by the data from Zerona,” said Dr. Kenkel, whose research focuses on laser treatments.

The lasers have been used for many years for managing joint pain. Already-released data from a previous study on Zerona reported that patients lost an average of 3.64 inches from their waist, hips and thighs combined. By comparison, the control group lost an average of 1/2 inch.

“To ensure you’re spending your money wisely to get the results you want, every patient should be evaluated by a physician first, and you should consult a board-certified plastic surgeon rather than just heading down to the closest spa,” said Dr. Kenkel, who is also chief of plastic surgery at the Dallas Veterans Affairs Medical Center.

“Just having the latest technology won’t guarantee you see the transformation you envisioned. As plastic surgeons on the cutting edge of technology, we can really give an objective assessment about what all the options are.”

When Cancer Is Dormant, Fat Stem Cells Safe For Breast Reconstruction

| September 20, 2010

 0 posts

Fat-derived stem cells can be safely used to aid reconstruction of breast tissue after mastectomy as long as there is no evidence of active cancer, according to researchers at the University of Pittsburgh School of Medicine. Their findings are available in Tissue Engineering Part A.

Plastic surgeons have long moved fat from one part of the body into the breasts for reconstruction, but with some complications and a varying success rate, explained senior author Vera S. Donnenberg, Ph.D., assistant professor of surgery, Pitt School of Medicine. More recently, they have considered adding stem cells derived from adipose, or fat, tissue (ADSC) or the bone marrow to the transferred fat with the aim of supporting graft integration by enhancing new blood vessel formation.

“But it has not been clear whether these stem cells are safe for breast cancer patients because they could send growth signals that promote tumor reactivation or provide new blood vessels for the tumor,” Dr. Donnenberg said. “Our research suggests that this risk is real if the patient still has active tumor cells, but is safe when the cells are inactive or resting.”

For the study, the researchers collected adipose tissue that would have been discarded during “tummy tuck” procedures performed by study co-author J. Peter Rubin, M.D., associate professor of surgery, Pitt School of Medicine, whose team has several federally funded projects underway to develop fat grafting and stem cell therapies for reconstruction of a variety of tissues.

The researchers isolated ADSC from normal fat and mixed them with human breast cancer cells obtained directly from patients. After two weeks in culture they found that ADSC greatly encouraged the growth of tumor cells. In a followup experiment, the researchers injected small numbers of highly purified active or resting tumor cells under the skin of mice either with ADSC or with previously irradiated tumor cells. The combination of active tumor cells and ADSC led to dramatic tumor growth, while injections of resting tumor cells were not affected by co-injection of either ADSC or irradiated tumor cells.

“There is already some clinical evidence that breast reconstruction with transplanted fat is safe. Our findings lead us to conclude that augmentation of fat grafts with additional ADSC should be postponed until there is no evidence of active cancer,” Dr. Donnenberg said. “Our data in the mouse suggest that dormant cancer cells are not sensitive to the growth signals sent by the ADSC.”

Benefits Of CyberKnife Lung Radiosurgery Treatments Emphasized During Europe’s Largest Congress In Radiation Oncology

| September 18, 2010

 0 posts

Accuray Incorporated (Nasdaq: ARAY), a global leader in the field of radiosurgery, announced today that for the first time in Europe the company showcased its latest offering, the CyberKnife VSI System, at ESTRO 29; the Annual Congress of the European Society for Therapeutic Radiology and Oncology in Barcelona, Spain. Also featured at this meeting were multiple presentations demonstrating this system’s best-in-class motion management capabilities for lung cancer care.

During a well-attended symposium on motion management in lung radiosurgery chaired by Pr. Volker Budach, M.D., Ph.D., Charite Universitatsmedizin, Berlin, physics and clinical findings were presented by representatives from two CyberKnife sites: Erasmus MC – Daniel den Hoed Cancer Centre in Rotterdam, Netherlands and Centre Antoine Lacassagne in Nice, France. Clinical research presented during the symposium demonstrated the benefits of CyberKnife radiosurgery in the treatment of primary early stage and metastatic lung cancer, including excellent disease control and preservation of quality of life. Enabling these outcomes are the CyberKnife System’s unique motion management capabilities including the Synchrony® Respiratory Tracking System, which enables real-time tracking and correction for tumors that move with respiration.

“The Synchrony System is unique in its ability to correct for changes in a patient’s pattern of breathing in real-time,” said Dr. Joost Nuyttens, M.D., Ph.D., Erasmus MC Daniel den Hoed Cancer Center, Rotterdam. “This allows us to significantly reduce delivery margins while eliminating the need for abdominal compression or breath-holding techniques.”

In addition, two CyberKnife users were recognized for outstanding research. The ACCURAY-ESTRO Award, which for the last several years has recognized radiotherapy professionals for original research in the field of high precision radiotherapy, was awarded to Dr. Joost Nuyttens (Rotterdam), for his lecture entitled “Outcome of Four-dimensional Stereotactic Radiotherapy for centrally located Lung Tumors”. Within the National Young Scientist Session, the first prize was won by Jean-Emmanuel Bibault, Resident, Centre Oscar Lambret, Lille for his oral poster reporting on outcomes obtained using the fiducial-free Xsight® Lung Tracking System titled, “Early efficacy and toxicity evaluation of CyberKnife Xsight Lung.”

“Both of these awards highlight industry-leading approaches to non-invasive lung cancer treatment using CyberKnife radiosurgery,” said Vittorio Puppo, General Manager, Accuray EIMEA (Europe, India, Middle East, Asia). “We congratulate our productive and dedicated users on this fine honor and thank them for their important research contributions.”

More than 20 posters and oral presentations during this year’s ESTRO meeting highlighted the continued expansion of extracranial radiosurgery applications with the CyberKnife System, including presentations on lung, liver, prostate, spine, pancreas, and head and neck cancer.

About the CyberKnife® Robotic Radiosurgery System

The CyberKnife Robotic Radiosurgery System is the world’s only robotic radiosurgery system designed to treat tumors anywhere in the body non-invasively. Using continual image guidance technology and computer controlled robotic mobility, the CyberKnife System automatically tracks, detects and corrects for tumor and patient movement in real-time throughout the treatment. This enables the CyberKnife System to deliver high-dose radiation with pinpoint precision, which minimizes damage to surrounding healthy tissue and eliminates the need for invasive head or body stabilization frames.

Materialise And Dr. J.B. Jupiter Explore The Power Of Virtual Surgical Planning In Correcting Compound Wrist Fractures

| September 18, 2010

 0 posts

Materialise announced that it would lend its considerable expertise in virtual surgical planning to researcher Dr. J. B. Jupiter, Chief Hand and Upper Extremity Service at Massachusetts General Hospital. Through an AO funded, IRB approved study, Dr. Jupiter will use SurgiCase(R) Orthopaedics to explore the advantages of computer-assisted surgery, virtual 3D planning and intra-operative use of patient-specific surgical guides in osteotomies to correct compound wrist fractures-the first research project of its kind.

Previous studies have proven a link between the quality of corrective surgery and overall post-operative wrist function. The challenge to the orthopedic surgeon is that the anatomical deformity associated with compound wrist fractures is complex and difficult to visualize via standard X-ray technology. While introducing 3D computer models obtained from CT-scanned images has helped doctors map multiple planes of deformity, they’re only useful during an operation as a reference.

To find out if SurgiCase(R) Orthopaedics can play a determinative role in the surgery’s outcome, Dr. Jupiter designed a randomized, multi-center trial of two groups of patients. All will be undergoing elective surgery to correct a compound wrist fracture. Surgical planning for the control group will be done through conventional methods, i.e. X-rays. Planning for the second (study) group will be computer-assisted.

That’s where Dr. Jupiter intends to rely extensively on SurgiCase(R) Orthopaedics. The first step will be obtaining CT images of a malunited and a contralateral normal forearm and exporting them to Materialise. There, clinical engineers will convert them to a 3D model and work with the surgeon to try out different approaches to the surgical plan. The model makes it easy for a surgeon to customize a plan for the individual patient.

Next, Materialise will build the surgical guides that carry the pre-surgical plan into the operating room. Custom-designed to fit the patient’s anatomy, the guides ensure the highest precision in drilling and cutting. The guide is constructed to fit a specific location on the patient’s bone and contains the exact positioning for the osteotomy and drill holes of the affixing plate.

After undergoing surgery, each patient will follow standardized protocols for post-op care. Outcome measurements will be evaluated under acceptable standards by the attending physician at regular intervals during a 1 year period.

ASAPS Applauds Florida Board Of Medicine’s Actions To Protect Patient Safety

| September 17, 2010

 0 posts

The American Society for Aesthetic Plastic Surgery (ASAPS) commends recent actions taken by the Florida Board of Medicine to protect the safety of patients seeking cosmetic surgery. The Board announced a possible one-year license suspension and $50,000 fine against a Florida family physician accused of allowing unlicensed medical assistants to perform liposuction, the second major disciplinary action recently taken by the Board that involves inadequate training in cosmetic surgery.

“Liposuction is a surgical procedure that should only be performed by a well trained and properly credentialed medical professional. Allowing unlicensed or unqualified personnel to perform this type of surgical procedure is a serious breach of patient safety. The recent actions taken by the Florida Board of Medicine send a clear message that this type of compromise to patient safety will not be tolerated,” said Felmont Eaves III, MD, President of ASAPS.

ASAPS urges all consumers who are considering plastic surgery to have their procedures performed under the care of properly credentialed and trained medical professionals. A doctor’s board-certification is the best indicator of his or her training in a particular medical or surgical specialty. Patients should select surgeons certified by the American Board of Plastic Surgery (ABPS), the only Board recognized by the American Board of Medical Specialties (ABMS) to certify doctors in the specialty of plastic surgery, and avoid those whose liposuction training is based only on the completion of a brief, weekend course. Patients should also be wary of physicians offering “deep discount” procedures, or those offering surgical procedures in non-clinical settings, such as medi-spas. More information on how to check surgeon credentials can be found on the ASAPS website.

Liposuction, also called lipoplasty and suction-assisted lipectomy, removes deposits of excess fat from specific areas of the body including the face and neck. It is the second most popular surgical cosmetic procedure in the U.S., with 283,735 liposuction surgeries performed in the U.S. in 2009, according to ASAPS.

Enriched natural oils combined with high oxygen creams are a reliable way to prevent UVA damage

| September 17, 2010

According to scientists, the major aspect of the damage done from UVA light are preventable by antioxidants. In studies these components have show significant protection and prevention by stopping the free radicals which inherently are the major cause of skin damage from the light of sun beds.

Such preventive components only come from natural oils and products which are made from just such natural ingredients. According to cosmetic scientists, Essentially Yours and many independent studies, enriched natural oils combined with high oxygen creams are a reliable way to prevent UVA damage.

It has been disclosed in recent months that oxygen has also been shown in mice as being a preventer to damaging skin from other light ie UVB.

So could products which are high in oxygen also help? All natural creams and gels not only contain almost total antioxidants but also unlike petro chemicals, naturals have that special small nano oxygen particles and if that was not enough, natural oils completely keep the skin moist and stop it drying out which is a regular complaint when using sun beds.

According to Essentially Yours, most sun bed related products are full of the very ingredients you want to avoid when using them and you should only use 100% naturals if your want to stay protected.

Essentially Yours will be at Olympia Beauty this weekend. The MD of the biggest beauty show once said “there’s few others around who have this scientific knowledge at their finger tips because they’re marketing companies not R&D Manufacturers like you” .

HydroCision Inc, a privately held medical device company, announced today the purchase of the majority of the Company by new investors, led by venture capital firms Volcano Capital, Bioventures, and Omega Funds. HydroCision is the developer, manufacturer, and marketer of SpineJet®, whose innovative Fluidjet surgical technology allows discectomy and interbody fusion procedures to be performed faster, less invasively, and with a higher degree of precision than traditional surgical modalities.

The parties also announced the appointment of Howard Donnelly to the position of President and CEO of HydroCision. Donnelly has more than 20 years of executive-level experience in the medical device industry. In addition to serving on several medical device company boards, he was the past President of Level 1, a medical device subsidiary of Smiths Group.

HydroCision will focus on several core strategies designed to strengthen its worldwide position in the spine market. “The global market for minimally invasive spine procedures is growing rapidly,” noted Donnelly. “This new investment and strategic direction will allow HydroCision to extend its technology leadership with a very strong product suite that will benefit both clinicians and patients.”

HydroCision plans to invest in U.S.-based research projects to further validate the clinical and economic value of HydroCision products for the medical community and the insurance industry. “We believe a stronger medico-economic justification for the use of our products will afford us the platform we need to scale adoption in the U.S. and OUS markets,” said Donnelly.

HydroCision’s SpineJet system has been widely accepted by spine surgeons as breakthrough technology for decompression and fusion procedures. Until SpineJet, spine surgeons were primarily relying on manual instrumentation to perform delicate procedures through increasingly smaller incisions. “The HydroSurgery® approach made possible by HydroCision’s technology provides spine surgeons with an efficient new tool,” said Mitchell Hardenbrook, M.D., an orthopedic surgeon at the Boston Spine Group in Boston, Mass. “SpineJet has empowered us to perform truly minimally invasive fusion procedures that lead to improved clinical outcomes and faster patient recovery.”

Does Your Insurance Company Know Who The Good Doctors/Surgeons Are?

| September 16, 2010

Several health plans have introduced physician rating systems to offer consumers more information when choosing their doctors. However, a recent study presented in the September issue of the Journal of Bone and Joint Surgery (JBJS) reveals that physician-tiering guidelines and results are not consistent across insurance companies, do not fully define quality; and could confuse consumers.

Since affordable and more accessible health care is a critical national challenge, the use of rating systems will increase as one response to rising costs. Doctors receive notices from insurance companies about their ratings, although they do not fully understand how their tier was determined. When patients receive letters from insurers and see that their doctors are not in a top tier, their reactions may range from disappointment to confusion. This study is the first to analyze tiering system data as it applies in a specific setting.

“We examined data on 615 orthopaedic surgeons who had been accepted in one or more health plans in Massachusetts,” explains one of the authors, orthopaedic surgeon Timothy Bhattacharyya, MD, Head, Clinical Investigative and Orthopaedic Surgery Section, National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS). “Tiering ranks physicians on the basis of cost efficiency and adherence to performance quality benchmarks. Thus, a higher physician rating could lead to lower co-pays and out-of-pocket costs for patients.”

This study also reveals concerns that should be addressed:

– Study analysis showed that board certification, Medicaid acceptance, and practice in a suburban location were independent factors associated with a physician ranking in the top tier. However, more years in practice or fewer malpractice claims, important indicators of quality, did not influence or factor into tier rating. Dr. Bhattacharyya and his co-authors suggest that tiering data deserves more exploration and a clearer definition of quality.

– Insurance companies did not agree on physician inclusion in top tier. Since ratings are not consistent from one insurance company to another, a physician is often rated differently from one plan to another.

– Patients need more information about physician tiering. While health plans display a physician’s tier in online and printed directories, explanation of stratification methodology and its relation to quality of care is limited. Currently, for example, there is no evidence to suggest that physicians ranked in a second tier provide lower quality care.

The authors note that their study has limitations. Data was examined for one specialty, orthopaedics, and in one state, and may not be applicable to other medical specialties or geographical areas. The lack of agreement between health plans may be the result of measuring different aspects of healthcare quality. A “gold-standard” consensus could serve as a benchmark and provide consistency across plans.

What should consumers do if they check a physician and discover that he/she is not in the top tier? Dr. Bhattacharyya explains that:

– Your physician’s rating may change from one plan to another, so compare his/her rating with another plan.

– When checking on a physician, ask about board certification and experience in this medical area or procedure. Seek referrals from people you trust, such as your doctor or someone who has had the same procedure or condition.

– Information about methodology and its relationship to quality of care is limited. Because this issue can misrepresent doctors and confuse patients, we should seek a universal definition of quality.
